Outreach Services We Provide
Assess needs and provide the necessary case management services as well as providing direct services of referrals that are required to deal with identified challenges.
Provide supportive services (filling out applications, gathering of verification material, transportation, item pick-up) if necessary to facilitate the client’s participation in the needed service.
Prepare income assessment forms and do preliminary screening to determine eligibility for other potential services.
Perform necessary advocacy and mediation services to facilitate enrollment into mainstream resource programs such as:
· Wisconsin Works Program (W-2)
· SSI, SSDI
· Food Share
· FSET Food Share Employment and Training
· Fuel Assistance
· Medical Assistance
· Section 8 Housing Voucher program
· Badger care
In addition to the many other mainstream resources available in our communities.
